So how exactly did Lily save Harry Potter from Voldemort?

I understood she sacrificed her life, so does that mean she took her own life before Voldie got her? Or did she do some old spell to use herself as a shield? And why was this the only time this had happened in wizarding history? I would ume some other mom in all of time would sacrifice herself for her child.
